Application Process:
The owner will make a final determination on the eligibility of the applicant based on income calculation, credit and reference checks. . In addition the community will conduct an initial screening and assessment to determine the prospective applicant’s service needs and preferences and the ability of Christopher Heights of Belchertown to meet those needs. Assessment will include applicant allergies, diagnoses, medications, dietary needs, history of psychosocial issues, lever of personal care needs, etc. Successful applicants will enter into a lease with the Management Agent and a deposit will be required. In order to be eligible, applicants must meet several threshold requirements:
• The household size must be appropriate.
• The applicant must have household annual income at or below the property’s income limits.
• The applicant must provide a birth certificate and social security card.
• The applicant must have a favorable landlord history of at least five (5) years.
• The applicant must have a favorable credit history.
• The applicant must provide a completed Physician Health Report. Applicants may request participation in Group Adult Foster Care or applicants may purchase services through Christopher Heights of Belchertown.
